QUESTIONNAIRE FOR SUPPORTING THE ACCREDITATION LEVEL

This questionnaire should be filled out annually by a School which has received the status of full accreditation returning it to the EAAA office

no later than October 5th..

The purpose of this questionnaire is to inform the EAAA Board of conditions at your School, your accredited program and any current changes in it.

Thus, we in earnest ask you to carefully analyze all aspects of your School’s life in regards to the standards and to the recommendation given by the accreditation team on their evaluate visit. The completion of this questionnaire on an annual basis is, in a sense, a “miniature” self-evaluation, repeating all of its stages.

1. General information.

Report all relevant information using the word «ATTENTION» to indicate changes over the course of the last year.

3.1.
