This questionnaire should be filled out annually by a School which has received the status of full accreditation returning it to the EAAA office
no later than October 5th..
The purpose of this questionnaire is to inform the EAAA Board of conditions at your School, your accredited program and any current changes in it.
Thus, we in earnest ask you to carefully analyze all aspects of your School’s life in regards to the standards and to the recommendation given by the accreditation team on their evaluate visit. The completion of this questionnaire on an annual basis is, in a sense, a “miniature” self-evaluation, repeating all of its stages.

1. General information.
Report all relevant information using the word «ATTENTION» to indicate changes over the course of the last year.

2. History and denominational affiliation
Describe any changes that have taken place over the course of the last academic year concerning your School’s denominational affiliation, relations with churches or with new churches and denominational unions. Do you have students applying from new denominations (confessions, unions, and so forth).
3. Stability, programs, goals
3.1. Describe any changes to the School’s goals, accredited programs, or Mission Statement. If there are new goals or a new Mission Statement, then write them below in their entirety.
3.2. Describe the programs the School is currently running during the current academic year. Enumerate on each program with particular attention to new ones. (For example, a program for preparing pastors, or for preparing Sunday School teachers, etc.)
4. School management
4.1. Have there been any changes in the School’s legal status. If so, please indicate these changes.
4.2. Have there been any changes in the administrative chain of command in your School? If so, then what changes have been made?
4.3. Describe any changes to the principles and procedures which govern the School’s leadership (Board of Directors). In an appendix indicate the chairman and a list of the Board’s members.
4.4. Has there been any change in the ownership of the land and property that the School uses? If so, describe the changes.
4.5. Do you have any future construction plans? In particular, describe any construction changes that have been made in the last year at the recommendation of the accreditation team.
4.6. Have there been any changes in the students living conditions? Describe all changes made, especially those changes for students in residence.
4.7. Have there been any changes in student tuition fees? Have there been any changes in educational sponsorship for the better of the students? If yes, please describe these changes.
4.8. Has there been any change in the way or size of student stipends, or in other forms of student support?
5. Academics
5.1. Indicate the number of students (including woman) in the accredited program for the current year.
5.2. Indicate the number of students (including woman) in each program for the current year.
5.3. The overall number of students
5.4. Have there been any changes in the composition or quantity of the number of years of study, subject matter, practical ministry time, and so forth, in the accredited program during the current academic year? If so, please describe these changes.
Append to this questionnaire the complete academic plan for the accredited program for the current year indicating all subjects studied and the quantity of lecture hours and homework for each subject.
5.5. Describe the changes that were made in the last academic years for the accredited program in connection with the recommendations made by the accreditation team.
5.6. How many students did your School graduate in the accredited program in the last academic year? Were there any changes in the form or contents of the graduation diploma? If yes, please describe these changes.
5.7. In the past year as well as in the current year: Did the accredited program accept students from other Schools? If yes, then describe how many and from which Schools.
5.8. How many national teachers have taught in your school in the last year?

5.9. What’s the number of guest teachers last year and in this academic year?
Append to this questionnaire a list of all teachers working in the accredited program for the last year and for the current year, indicating their gender, citizenship, language of instruction, degrees held, knowledge of the area of teaching or subject which they taught, teaching time each week, and any other additional obligations, such as preaching ministry, etc.
5.10. Describe any changes that have occurred in the last academic year in regards to the teachers workload. It is especially important to note how faculty meetings went and what changes were put into effect as a result of recommendations made by the accreditation team.
6. Library
6.1. Indicate the number of individual titles in the library collection
6.2. Indicate the number of individual titles in the language of the students
6.3. Describe any changes in the way the library is operated. How does the library council work? Who has joined this council? How many books were purchased during the last academic year, including books in the language of the students? How much was spent on purchasing books and on library development, in U. S. dollar equivalent?
6.4. Indicate the number of individual theological periodicals
6.5. Describe how the cataloging and computerization of the library is going, whether or not the students can use the Internet to access the catalog, and how they use it.
6.6. Describe any changes in the way that the library is used, how many hours per week the library is open, and how many books have been checked out by students in the preceding academic year.
7. Ministry and spiritual formation
7.1. Describe any changes in the conducting and control of the students’ practical church ministry? How many hours per week are students of each program involved in Christian ministry? How many hours of church ministry do students in the accredited program have each week?
7.2. Describe how the School is actively involved in the development of the personal, spiritual life of each student. What kind and how many worship services are held in the School and have there been any changes in this area?
